Assessment & Planning
Risk assessment, needs analysis, and matching to appropriate property type and location. Multi-agency planning meeting if complex needs.
Move-In & Orientation
Supported move-in with property orientation, community introductions, and essential service connections. Clear contact information for ongoing support.
Ongoing Support
Regular check-ins, crisis support, care coordination, and progress reviews. Move-on planning when residents are ready for more independence.
